Block Level Officers (BLOs) have started visiting voters with enumeration forms as part of the ongoing Special Intensive Revision (SIR) of electoral rolls.

Several doubts and confusion prevail over filling up the forms. This article attempts to simplify the process of furnishing details on the relevant fields with the help of the examples of three voters.

To make it simple, let us divide the enumeration form into three parts: A, B and C. (Please note that enumeration form will be provided to only those whose names figure in the 2025 voters' list).

Ramesan: His name was included in the 2002 voters' list. He needs to fill up only parts A and B of the enumeration form.
Suresan: His name was not in the 2002 voters' list, but his closest relative (father Madhavan) was included. Suresan needs to fill up only parts A and C.
Dinesan: Neither he nor his relatives figured in the 2002 voters' list. He needs to fill up only part A.

Things to remember
Filling up the enumeration form becomes easier if Ramesan finds his name and details on the 2002 voters' list. In case of Suresan, he has to find his father Madhavan's details.

To find the details, go to ceo.kerala.gov.in/voter-search, and search using the name of the district, Assembly constituency, booth and name (The constituency and booth are of 2002). The search words should be keyed in Malayalam. The Google tool to facilitate Malayalam typing is provided on the website.

If you cannot remember the booth, search using the name of the voter. If you find the name of one voter in your house, the remaining names and details would be available on clicking on the house number.

Once you find the details, note down the name (as in the 2002 list), ID card (EPIC) number, relative (related person), relationship with the person, part number, and part serial number. If you find the search difficult, seek the BLO's help.

Dinesan needs to keep his present voter ID (EPIC) number and Aadhaar card.

Filling up the form

Part-A
(Applicable to all three scenarios of Ramesan, Suresan and Dinesan).

The latest details have to be furnished. Besides your date of birth, Aadhaar number, and mobile number, provide the names of your parents and spouse. If you know their EPIC (current, not that of 2002) numbers, provide them too. These numbers could be found in the new voter ID card or the voters' list of 2025.

Dinesan needs to fill up only part A. He may produce the relevant documents proving his eligibility upon receiving a notice. The notice will be issued after a draft list is published.

Part- B
(Applicable to scenarios similar to Ramesan)

To be filled up by those included in the 2002 list. All details must be as same as in the 2002 voters' list.

  • Voter's name: The name should be the same as the one in the 2002 list. If R Ramesan was entered as 'Ramesan', the name should be entered without initials.
  • EPIC number: The number should be that of 2002, not the latest one. Some voters did not have the number in 2002. They may leave the field blank.
  • Relative's name: The name mentioned under 'Relative" or 'Related Person" mentioned in the 2002 list must be provided. (E.g. Name of Ramesan's father, Gopalan).
  • Relation: Relation between Ramesan and the 2002 voter Gopalan. Write 'Father'. If the relative is Ramesan's mother, write 'Mother'.
  • Name of the Assembly constituency: The constituency where the relative lived and cast his/her vote. The name of the constituency and its number might have changed.
  • Constituency number, part number, serial number: These numbers will be given against your name in the 2002 voters' list.

Part - C
(Applicable to scenarios similar to Suresan).

Name not in the 2002 list, but close relatives were included.

  • Voter's name: Remember, it is not your name that has to be provided, but that of the close relative included in the 2002 voters' list (Father, mother, grandparents, siblings). In Suresan's case, he has to mention his father (Madhavan)'s name, which was in the earlier list. The close relative's name could be provided even if he/she is not alive.
  • EPIC number: Suresan has to provide his father, Madhavan's 2002 EPIC number, not the new one.
  • Relative's name: The name of the relative mentioned against the voter (Madhavan)'s name in the 2002 list should be provided. In this case, the name will be Bhaskaran, Madhavan's father.
  • Relation: The relation should be the one between Madhavan and Bhaskaran. Note, it is not Suresan and Madhavan.
  • Name of the constituency: The constituency where Madhavan was a voter in 2002.
  • Constituency number, part number, serial number: These numbers will be found against Madhavan's name in the 2002 voters' list.

Points to note
Glue your recent photograph only if you wish to change the existing photo in the list.

A voter should fill up two forms. One should be handed over to the BLO, and the other one should be kept by the voter as an acknowledgement after getting the BLO's signature.

Sign with the date below the form. If the voter concerned is not at home, any elder relative could sign by specifying the relationship.

Use the 'Fill Enumeration Form' option on the website, voters.eci.gov.in, to fill and submit the form online.
